BAT (Basophil Activation Test)

Basophil Activation Test (BAT)

The Basophil Activation Test (BAT) is one of the most advanced and innovative precision diagnostic tests in the field of allergology. Performed in a laboratory setting, it evaluates the functional response of the immune system, providing valuable information in complex diagnostic cases.

When Is the Test Recommended?

BAT can be particularly valuable in cases involving severe or potentially dangerous allergic reactions, including:

  • Drug allergies: When further assessment of suspected allergy to antibiotics or other medications is required without directly exposing the patient to the suspected drug.
  • Hymenoptera venom allergy: In patients with a history of systemic allergic reactions or anaphylaxis following a bee or wasp sting.
  • Food allergies: Particularly when conventional diagnostic methods, such as skin tests or specific IgE testing, produce borderline, inconclusive, or conflicting results in relation to the patient’s clinical history.

How Is the Test Performed?

The method combines advanced laboratory technology with a high level of safety for the patient:

  • Simple blood sample: The procedure begins with the collection of a small blood sample.
  • Laboratory simulation (ex vivo): The patient’s white blood cells—specifically the basophils—are exposed to the suspected allergen under carefully controlled laboratory conditions.
  • Immunological analysis: Using advanced flow cytometry techniques, specific activation markers on the basophils are measured to determine whether the cells become activated in response to the suspected allergen. This provides a functional assessment of the patient’s allergic response.

Benefits for the Patient

  • High Level of Safety: The patient is not directly exposed to the suspected allergen during the test, as the reaction is assessed exclusively in the laboratory. Therefore, BAT itself does not trigger an allergic reaction or anaphylaxis in the patient.
  • Reduced Need for Potentially Risky Challenge Tests: In selected cases, BAT can help reduce the need for direct diagnostic challenge testing, particularly when the estimated risk of an allergic reaction is high.
  • Greater Diagnostic Clarity: BAT offers high specificity in appropriate clinical settings and can provide valuable additional information when conventional allergy tests are inconclusive, helping the allergist reach a more accurate and individualized diagnosis while minimizing unnecessary risk.
